In almost all cases of transgender it makes it appears at young age often before puberty. The child may or may not adapt. Those that can't end up in some form of therapy. Those that can adapt do in various ways such as cross dressing, engaging in homosexual relationships, assuming the role of their gender or even totally rejecting their sexuality. However, at some point in their lives something happens and they find themselves having to confront the same problem they had as child and adaption does not work for them.

However, even knowing that gender cannot be changed, therapy is used as encouragement that gives false hope that it can. Keep feeding kids with hormones and telling them Cinderella is real, instead of facing them with reality. Once they face the reality on their own, and realize there are no unicorns, what's left for them?
