If a doctor serves as the physician to any president, imo, that requires him to not make any speculative statement about any other. I think you'd be giving up some freedom to take the job, just as most of us do when we take jobs.

The debates were almost two years ago. There is a clear decline since then. It can be seen when he speaks, when he is wondering around looking lost. He can still be lucid, but those times are growing further apart.

imo he sort of drank the Trump koolaid, and got bitten by the political power drug .... but ... I just don't recall any presidential dr going political before. Being potus seems a highly strenuous and mentally taxing job. Biden's not up to it, but neither was Trump imo.
Trump had McConnell and Ryan who know how run their majorities, while Biden was saddled with schumer and Klaine who torpedoed Pelsoi
And I recall Reagan having to bring back Howard Baker to finish out his second term and keep him looking lucid
