The question is whether their preferred gender reflects that biology.

If I told the doctor that I preferred to be black instead of white, he would likely say that is nice, but that doesn't negate the fact that you are actually white. I could live as an black person, whatever that means, but I don't believe it makes any sense for my doctor to recommend having life altering plastic surgeries to affirm my preference nor for him to affirm my belief that although I was born white, I am really black.
