I clicked all four, although I am a widow. I never un-married my husband, although my church might think differently.

This is why widows continue to wear their wedding rings, IMHO.

Baptized in a lake at 12 years old I was judged to be at an age of reason. Baptized by the Holy Spirit as a teen.
Rededication Baptism in the Church Baptism pool and accepted into the congregation.
Participate in the Lord's supper when at church.
I am not sure what is meant by confirmed.
Still single my whole life.
baptized in a lake!
that is special!

I was married when I was very young, to a Mississippi man, whose brother was a Baptist minister. He baptized his brother multiple times, usually in a lake or a stock pond. (A stock pond is a type of pond that is typically man-made and is "stocked" with fish, meaning a population of fish were imported and developed in the pond.)

MULTIPLE TIMES!

In The Church of Jesus Christ of Latter-day Saints, baptism occurs after you reach the age of accountability which is 8 years old. We baptize by complete immersion in water by one who holds the priesthood of Melchizedek or Aaron. Upon being baptized we then receive the gift of the Holy Ghost by the laying on of hands of one who holds the Melchizedek priesthood. At the time we receive the gift of the Holy Ghost we are also confirmed as members of The Church of Jesus Christ of Latter-day Saints so this is what we deem as receiving our confirmation. Baptism and receiving the gift of the Holy Ghost is a covenant we make to follow Jesus Christ in our lives. We renew these covenant every Sunday by partaking of the sacrament of bread and water. I surmise that Holy Communion is similar to partaking of the sacrament. The sacrament can be blessed by either a Melchizedek or Aaronic priesthood holder. We also not only believe that we can be married in this life but also believe that through the Melchizedek priesthood we can be sealed in marriage for time and all eternity. We believe that is part of the priesthood power that Jesus gave to his disciples:

Matthew 18:18
18 Verily I say unto you, Whatsoever ye shall bind on earth shall be bound in heaven: and whatsoever ye shall loose on earth shall be loosed in heaven.

The power to bind marriages for all eternity is part of the priesthood power of the Melchizedek priesthood.
