Just saying it looked like she was outside on public property. I’m no lawyer but I’d assume trespassing requires actual, uh, trespassing.

Trespass and obstruction. Both are crimes, when the owner asks you to leave and you refuse, that's trespass and if you refuse to leave and argue with a cop, that's obstruction.

You asked who makes the decision, so I answered you. Anyone who is a representative or owner of that establishment can trespass someone from that property.

They can be trespassed because the manager doesn't like their shirt. Or because he doesn't like their hair. Or their voice. Or he can simply have them trespassed for no reason at all. When you are issued a trespass warning, you are not given a reason, nor is it required.

If the manager "trespasses" Black people because he doesn't like them, but he doesn't give that reason, it's OK?

It is likely she makes them not charge her for the drinks or she will take their license away. Like in that True Grit movie where he says I confiscate that about buying liquor. That is why he doesn't want her in there.
