I really don't want to be here anymore but there is no other place to go in the world where a person can go and be as 'free' so I'm stuck here. In an ideal society - one with all that super duper freedom of god and stuff you just described - when a person shows up in the emergency room and needs immediate treatment to save their life - and they don't have any proof that they have insurance, nor enough cash on them for the service, they should -
a) Die.
b) Have their lives saved at the expense of the hospital
c) Have their lives saved at the expense of the taxpayer
d) Have their lives saved at the expense of the hospital's paying customers.
e) Be saddled with so much debt they can never pay it off and hence something between b, c, and d inevitably will happen.
f) Have their lives saved at the expense of a magical tree that grows money which exists in your ideal world
