Why would he have a microphone boom attached to his ear when there is a microphone on the podium? The box in his jacket is much more likely a receiver for the ear piece. Someone guided him through is speech. So what? Most politicians are mouthpieces for people behind the scenes who aren't as photogenic, charismatic, whatever. A wireless mic doesn't go anywhere near the ear. That's for somebody to talk to him from offstage. There's no other purpose it could possibly serve. A mic would clip on his tie or lapel (in front, otherwise it would be entirely off-axis) and run down to his belt where the transmitter would clip.

If he were a musician he might need a stage monitor to follow (e.g.) what another signer is doing, and that could be an in-ear device. But for a single audio source (speech) there's no such need.

I know a few people who work behind the scenes in cable television. All anchors wear this piece and get prompts and info from their producers. Anchors are known as "the talent", not the brains.

The one Cruz is wearing is very sleek, almost hidden. I'm sure he didn't want that close up taken. It gives him away completely.

In working with a producer/IFB, follow the same steps:
  1. 1. Communicate your expectations. This means identifying in advance what the producer needs to tell you over IFB and what words/phrases you should expect to hear. Will he/she give you cues on when to start speaking? If so, agree on what the exact wording will be said in your ear, such as “go,” “now,” “cue,” or etc. Does the producer want to tell you how much time is left in the segment? Agree on how often you need to hear it. Generally, you will want a cue that there is “one minute” left, then “30 seconds,” then “ten seconds,” and “five.” Also, determine what other information the producer may need to tell you, and agree on what words/phrases the producer will say to communicate it. If the words are expected or familiar, you won’t be thrown off when you hear them.

And if you do wear a wireless mic for the purpose of amplification and/or recording -- it never ever goes anywhere near the ear. That would NOT work. At the best you'd get a hollow, distant muddy voice. More likely you'd get tremendous howls of feedback.

I didn't opine on what they were feeding him, and if you've ever tried to speak in public or on the air while someone's talking in your ear, "word for word" wouldn't work anyway. That's what teleprompters are for. What would work is to feed quick cues of talking points, reminders of something not yet covered, that sort of thing. The quick condensed kind of stuff you might, say, write on your hand if you didn't have this technology. Bush's apparatus would have been used the same way. It's cue cards without the visual distraction, but with an offstage feeder you get the advantage that somebody else -- maybe several people-- can think of these cues for you. Thus it becomes a sort of speech-by-committee, the purpose of course being to create this automaton image that you can sell like a commodity. Which is, after all, what political campaigning is these days.

The poster described how offstage directions and cues are fed in a newscast, and it's exactly accurate. Again, it's in "Broadcast News" in great detail, and that movie is very accurate in its depiction of what goes on in a studio. I would have posted a sample clip for illustration but YouTube doesn't seem to allow any to be hosted.

Anyway -- when you're narrating a script or speech formally the delivery becomes more about the sound and the cadence than the content. We use a kind of "mental buffer" that keeps the next few words in quick-storage while another part of the mind may be putting the next packet together -- or in this case may be listening for cues on what to say next. Meanwhile the speech is delivered uninterrupted. If you can keep the mental buffer floating evenly, no one in your audience catches on. It's really little different from being in a conversation with several people, and while you're talking someody over on the side chimes in, and you pick up on it.
