1) They didn't go to the wrong address. They went to exactly the address they intended to, for exactly the people they were looking for, who were actually there.
2) Taylor and her boyfriend were apparently standing right next to each other at the end of a hallway. It's not really that surprising that she got hit when two officers fired at her boyfriend.

In Kentucky, both voluntary and involuntary manslaughter charges require that the person charged be doing something illegal when the death occurred.
