First, Biden wasn't testifying and taking the fifth. That was his doctor. Second, he could easily have answered truthfully without revealing patient information. "Were you told to lie about the president's health?" is about whether he was pressured to lie, and he ducked it because he thought answering it would get him in trouble. Pleading the fifth is not a refusal to testify. Pleading the fifth is refusing to answer a particular question because by answering it truthfully, someone could put themselves in legal peril. "Were you ever told to lie about the president's health?" There are two possible answers, yes or no. If no, there's no need to plead the fifth because you don't get in trouble if you told the truth. If yes, you can get in trouble because you did. Obviously, he believes he would get in trouble if he admitted he was told to lie.

No, the purpose of the fifth is to allow you to not incriminate yourself. "Did you rob the bank?" is a perfect use case for it, because if you did and you answer truthfully to avoid a perjury charge, you will be admitting to a crime and incriminating yourself. What possible incriminating could the doctor be doing by answering the question, "Were you told to lie about the president's health?". Get that resolved and you'll see why he invoked it.
