Watch closely.

The kid thumbs the hammer but the cylinder didn't look like it moved into the correct position.

It fires while the cylinder is out of alignment with the forcing cone.

Basically the bullet fired into the frame and thru the side of forcing cone which then catastrophically destroys the barrel.

That's my guess.

I went back and watched it several times.
It looked like it went to the next chamber to me.
And if was to far off the hammer wouldnt have hit the primer.
