They go to the website and sign up. healthcare.gov. There is no enforcement of the legal status requirement. Check the American citizen box and they're in.
Not to mention those who jumped the border and were given a court date upon apprehension have "legal status" and are currently fully eligible. the BBB ends that starting in January, which is the real reason that the democrats have shut down the government.
They don't care about people's out of pocket premium costs going up by a couple of hundred dollars. if they did, they would have never passed the ACA in the first place.

When I was hospitalized, the other person in the room was a non-English speaker with a back problem. They used a phone translation service to communicate with her. I heard them tell her they wouldn't treat her other than letting her lie on her back until she could leave.
Also, they wouldn't treat someone at all who claimed to have a belly ache.
I don't know who pays for their semi-treatment.
