Most folks agree, that Jefferson was carrying on a relationship with her half sister for many years. I do not know if that continued in his presidency, but considering how he treated their (alleged,) offspring, and how the Jefferson family looked after Sally after Thomas' death, it is probably a safe assumption that their relationship continued while he was president.
There are no known, and likely were never made, portraits of Sally.
However, accounts are, she was indeed a beauty.

Perhaps, if Jefferson and Sally had lived in the twentieth century in a time where relations between folks of any ethnicity are widely accepted, and had he been able to marry her and been in office in our day, indeed, Sally might have been the most beautiful first lady this nation had ever seen.
