They aren't pleasing him enough. he wants even lower rates which might make the economy look better in the short term but puts it on shakey ground. The earlobe is on the bottom of the ear, not the top. He was grazed by the bullet on the top of his ear, which was fired in an attempt to assassinate him.

The blood was obviously real. And the other shooting victims were also real.
