Imagine how uncomfortable one would have to be in order to do all that to feel better about who they are. I never had the desire BUT if I had a terrible accident down there or developed cancer down there I might consider it as an option. You never know what life will deal you in the next hand. Hormonectherapy must start before the onset of puberty for best results. Most surgeries don't occur until the late teens. When they are finally ready..
