Not afraid to answer a self-evident question, but since you're fixated on a particular answer and you'll dismiss the self-evident anyway, this is for the adults:
The money comes from the insurance company, when the patient has insurance.
In the past, many people couldn't afford insurance.
Even people who had insurance could find out their insurer refused to cover their treatment, or put a lifetime cap on payment for that treatment.
Since passage of the PPACA, more people have insurance now.
And since passage of the PPACA, insurers are not allowed to refuse coverage for preexisting conditions, or put lifetime caps on reimbursement.
