You're thinking of Medicaid. Ryan's plan gives states a block grant to spend on Medicaid as they wish. And yes, Ryan's plan does effectively eliminate Medicare. And at the same time, it gives the rich more tax cuts and doesn't touch defense spending. Not serious.

Medicaid is a welfare program that provides healthcare for the poor. It is a state program paid for jointly by federal and state funds. Medicare is a healthcare program that you pay for through payroll taxes. Unlike Medicaid there are no income limits. Part B Medicare pays 80% of doctor fees for seniors but requires monthly premium. Medicare is a government operated health insurance program subsidized by the government.
