When jumping into an abyss it's best to double and triple check that everything is as should be.
This is no time to assume you've been given the okay signal when you haven't.

Or don't leap off a 164 foot fixed object to begin with. That would be my advice to anyone looking
for a thrill.

Probably was. She should have personally checked her equipment instead of treating the jump like an amusement park ride. You must know your limits and know how to check your equipment and then take time to check your equipment in high impact sports, or you may impact in a way you did not anticipate.

Those tend to be precisely the group that's easiest to force compliance on. She made a dumb mistake and she lost her life for it. It's a tragedy and the business will be beggared if they weren't following their own policies and procedures. More so if those procedures weren't clear, to begin with.
