Watch-based Heart Rate Monitors

I have had a couple different "fit bit" products that purported to monitor my heart rate, but they varied between being merely inaccurate and being very inaccurate. Now I have a Garmin thing that is good for walking, distance, time, riding, etc., but the heart rate monitor is not good.

Anyone have any good experience with one that actually monitors heart rate accurately?
 
No, I have no recommendation- IDC about mine. When my time comes it won't matter anyway- and at 73 that could have been 15 min ago-
 
I have a heart condition that occasionally causes my heart rate to elevate to dangerous levels, and I have to back off from what I'm doing (e.g., pulling a steep hill on my bike), but I cannot sense whether I am experience normal stress from exertion or such an incident. Is my HR 120 or 190? That's the difference. It would be nice if I had a device that could tell me in real time what my rate is.
