Not sure about the US, but normally visas are issued per sector where there's a skills gap, plus the quantity varies based on numbers required. This means, you can't hire over your own people because there's not enough to fill the jobs in the first place. So you might have plenty of visas requiring doctors, but none for being able to wash cars with a sponge.

Illegal immigration and free movement of people ***** it all up, it makes the poor poorer, and the rich richer.

So I don't know if you're either confused or you just don't know how visas work.
