I just watched my first one and remembered it, frickin grusome as I read about it in the Chicago Sun-Times they found just the pilots face in a mud puddle.

Jesus Christ, the cause of this one was because 1st shift maintenance removed the screws and 2nd shift maintenance didn't reinstall the screws on the airplane..
