Actually, evidence doesn't support this "lack of oversight". It depends on individual state rules. As it is, what the evidence supports is that fatalities associated with abortion are extremely rare. Early abortions have almost zero complications and until you get into the third trimester, complications and mortality rates are still well below full term pregnancy and child birth. Even in the third trimester - they don't exceed natural childbirth, but are in line with them. Of course, third trimester abortions are almost always for the life or health of the mother or severe fetal deformaties.

Medically necessary abortions always have taken place in hospitals, and nobody has ever had an issue with them.
