Could we see off shore ships for medical care?

Just postulating. During different times in US history, off shore ships have served the public. There were gambling ships complete with an army of whores to keep the male customers happy. Then there were offshore radio broadcasting ships.

Now, I am wondering about an offshore medical facility for the more affluent in our society who would think nothing of taking a helicopter from Boston - Washington (Bo Wash) or San Francisco - San Diego (San San) to ships located outside our declared limits where outside the US medical plans could cater to their needs.

If the medical facilities are outside the US they can not be taxed. Right? (I am asking because I am not absolutely certain on this.) This might be an interesting thread if there are any legal minds posting on this forum.
