The crew of the helicopter was contacted twice by the tower and twice confirmed the had visual contact with the landing aircraft and twice requested to take responsibility for visual separation.

I can't see what else the tower could have done.

The passenger jet did nothing wrong. They, to use the layterm, had the right-of-way.

Normally, the pilot not flying would be the one on the radio with air traffic control.

I'm pretty sure she was the one flying.

Both her as a ''co-pilot'' and him as the ''pilot'' have flown out of there hundreds of times, regardless of their designation.
