NTSB says entire crew in violation for sleeping before fire

A ship at sea, not at an anchorage, is required to have a deck watch or lookout.

International Regulations for Preventing Collisions at Sea (1972) COLREGS Rule 5 says in part:

"Every vessel must at all times keep a proper look-out by sight, hearing and all available means,in order to judge if risk of collision exists."

Great annotation. Too bad that these divers would have been breathing compressed air and not pure O2. That said leaking pure O2 is completely harmless and escapes to the atmosphere, it could only add to a fire if it was fed directly into a fire, and if pure O2 was there in the first place. However it is a common mistake believing that divers would be breathing pure O2

Wrong a ship anchored with passengers must also maintain a lookout.

Or are you saying that the coast guard does not understand maritime law?

The report indicates that there was no night watchman roaming the three-deck vessel when it caught fire, which U.S. Coast Guard officials said is a maritime requirement.
