My neighbors three doors down are from Australia. The Man (mid-late 30's) is one of the better mechanics I've seen and his wife is an RN. Their Son is an athlete and all-around good kid. Their daughter is a beautiful child.

The goobermint is forcing them to leave. I talked to him a couple weeks ago and the whole family is upset. I mean, seriously upset. His wife seems to constantly be in tears and the kids are on the verge of panic. They've all been here for over ten years.

Something happened with the Visa. Don't get it. Something about work -- Whatever. But they're being forced to leave.

Meanwhile we've got thousands and thousands of beaners living in shitholes, driving like idiots (one of them just ran over and crushed my SIL. Wrecked her back, she is essentially crippled for life. Again, we write off millions in unpaid bills every year. Don’t get me wrong, we make shitloads of money off of insurance. The uninsured show up here too but they are not really the big drag. The big drag we have is the $8,000 bill after insurance and we see about $400 of it. 5%.

I think that if we want to solve problems we have to actually address the root problems of them, not settle for superficial solutions that merely shift the problem somewhere else.

Our country has a physician shortage. We also have a nursing shortage. We can't solve the problem by trying to transition into a nursing based system of primary care. All that will happen is that we'll have an even wider nursing shortage.

Nursing shortages are due to mostly the same causes as physician shortages. There aren't enough available slots in nursing schools to educate sufficient quantities of students to keep up with demand for the labor. This causes the work to be all the more stressful, and subsequently leads to increased departures from the labor pool.

Even if we pour an infinite amount of money into additional education for existing nurses to move into NP roles so that the system transition into primary care nursing instead of primary care medicine, we're still going to have an ever increasing shortage of primary care providers.

But chances are that shortage will accelerate. NPs are already in high demand. Many hospitals will provide full tuition reimbursement for nurses to go back to school. But the demands of doing the job under perpetual labor shortages drives people out instead of up. Increased strain on the nursing system caused by transitioning to a nursing primary care model could become a disaster.
We've already transitioned.

A lot of people put the blame on schools being unable to push out enough people. That's definitely a big part of the problem. But that really means that it's the educational system as a whole that is failing our country. We need more medical schools. We need more nursing schools. We need them to be accessible. And we need a greater emphasis on training people to do boots on the ground care positions, as opposed to elitist and corporatist research positions.

Also, we need to abolish health insurance outside of catastrophic care, so that the private health care sector can become a healthy market once again.

Affluent people will be able to retain a personal physician through exclusive “concierge medicine” services. But here’s what others can expect: routine visits with a rotating cast of nurses and physician assistants with increasingly spare and online checkups with doctors.

Indeed. I'm not sure we need to ban it, but we should certainly repeal all the tax and regulatory policies that prop it up. We shouldn't be relying on insurance for routine health care.

I have a person physician and a physician group. I am certainly not rich. I just made a conscious decision years ago. I can usually get in to see them on short notice, they have full admitting authority if I need it. It is really rare for me to go to a convenient care facility, and then only if I am losing blood, somebody needing to give me a stitch in time, or am injured in another state on some mountainside. I have not been to an ER since I retired from the military, back in the late 90s except for my last skiing accident. Everything goes through them. The get copies of all tests and review them. They recommend my specialists and surgeons. My healthcare is covered under Medicare and Tri-Care and it is rare for me or my wife to see a bill that comes out of my pocket.

If you do not have a primary care physician, you have learned to trust, that has all your records, even military treatment records in whatever country, going back 40 or 50 years, you should consider making a change.

I'll be around thirty more years, get used to it.
