What over-used, or wrongly used words and phrases annoy you the most

"Issue" used as noun meaning "problem" rather than "concern." Many things concern me, but unless and until I delve into their nature, whether the things that concern me are indeed problems remains to be seen.
 
The word "so" when used to start a sentence.

A lot of people start every other sentence with the word "so" even though they're not tying a previous sentence to the current one. I haven't found any rule saying it shouldn't be done, but it's irritating as hell when it's overused.

( In response to a TV anchor thanking the guest for being on the program. ) I would simply say, "Thank YOU," or "my pleasure" or you're welcome."

It all takes up too much time. If the anchor has to thank someone for being on their show I would like for the anchor to go right into the interview instead of waiting for the time-consuming and cliché response.
