Seems to me the earnings from the Casino should be able to build, staff and run a hospital.

Unfortunately, most enrolled members of Indian tribes don't get anything like that amount.
CTUIR members receive dividends which amount to about a thousand a year, and they have a very lucrative casino.

If Indian health clinics go to fee-for-service they won't see much traffic. And Indians will head back to the state to get medical cards.
