1) On a per capita basis, we are only slightly worse off than your Garden of Eden country.

2) When I asked the pharmacist about it a couple weeks ago when she was administering my wife-mandated travel vaccines, she said that they don't even carry the measles jabs since the vast majority of americans get them from their pediatrician when they are wee ones.

3) I assume these outbreaks are among immigrants or kids with kooky parents but IDK as I have not seen any clear data on it. Ours have had theirs; I've had mine (twice). Not much more I can do to stop the outbreak. I could stockpile Calamine lotion and sell it for 5 times the price online though.....

That was debumked years ago. I remember being dubious about mmr myself. But my wife just waited until I went to work and got the kids jabbed. Im grateful she did.
