Hospitals don't do drug tests as standard procedure. A lot of people refuse to go to the hospital when being treated by medics. Most everybody hates being in a hospital. He seen his doctor the next day for further advisement.

Well for one they wanted to find out if narcotics played a role in his violent attack. They do that to all suspects who get killed committing a crime like Trayvon was. There was no need to test Zimmerman. The police seen no conduct by him which would give them a reason to suspect he was intoxicated. In self-defense cases where the evidence is crystal clear, there is no reason to drug test the victim.
