JFK Jr and his wife Carolyne Bessette and her sister Lauren Bessette were killed when the small plane he was piloting went down July 16, 1999.

He took off at 8:30 pm for a 90 minute flight to Martha's Vineyard where he was dropping off Lauren Bessette. Then he was to continue on to Hyannis Port, Mass for his cousin Rory's wedding.

It was gross negligence on Kennedy's part. He was only certified for visual flying. He was not instrument-rated. By taking off at dusk, he was guaranteed to fly in darkness, trying to rely on visual cues. As long as there were lights along Long Island Sound, he was OK, but once he got away from the lights into total darkness on a hazy evening, he had no chance. How could he have taken such a blatant? My guess? He was rich, good-looking, and indestructible in his mind. He was master of the universe, and had the adoration of woman all his life. Everything worked out for him always. in addition to making a foolish decision, he also had his radio on the wrong frequency, one digit off, and therefore had no radio contact with Air Traffic Control.

Kennedy ignored his instrument panel, particularly the screen showing his position relative to the horizon. Sans instruments, small planes often tilt imperceptibly. Your body doesn't feel it. It was thought Kennedy became disoriented and didn't trust his instruments. When he tried to correct according to his "feel", he was in deep trouble. After veering south, climbing for a bit, then leveling off he nosedived into the ocean upside down just short of Martha's Vineyard, The three were found five days later still strapped in their seats.

The women:
Carolyn Bessette was 33 years old. She was a natural brunette, but Kennedy liked his women tall, skinny, blonde, with their hair pulled back tight. I thought Carolyn looked hideous;. His decision to fly at night is hard to fathom. He either didn't understand the risks or foolishly chose to ignore them.

Not only that, while he habitually flew over water, he chose to fly a single engine plane. That left no margin for error in the event of an engine failure. He had plenty of money to get a twin and a multi-engine and IFR rating but apparently couldn't be bothered. Like a lot of rich people, he believed that Murphy's and Darwin's laws didn't apply to him.
 
If he had survived he might have been charged with the negligent homicide of his wife and sister in law.

My instructors were diligent in teaching pilots like me the dangers of night flying over the ocean. Kennedy no doubt was in a hurry.

He had indeed gotten some instrument training but hadn't completed it. I read where he had done the same flight 14 or so times and at night three times. Perhaps the difference the final time was haze, although he was listing rightward for some time and apparently didn't notice it. That may have been the curveball that threw him, taking him away from the coastline lights.

Seems a clear case of spatial disorientation.

The official investigation by the National Transportation Safety Board (NTSB) concluded that Kennedy fell victim to spatial disorientation while descending over water at night and lost control of his plane. Kennedy did not hold an instrument rating and therefore he was only certified to fly under visual flight rules (VFR). At the time of Kennedy's death, the weather and light conditions were such that all basic landmarks were obscured, making visual flight challenging, although legally still permissible
