The Pennsylvania Police Commissioner explained what went wrong pretty clearly.

It was the responsibility of local police to monitor the roof that Crooks fired from.

Two local Police counter snipers had clear visibility to the roof that Crooks was on, but when they heard reports of a suspicious person, they left their positions to try to locate him.
