I would have no problem if we got rid of "Not guilty by reason of insanity" and replaced it with a more sensible "Guilty, but insane".
The reality is, unless you have really good lawyers, few people get away with an "insanity plea". (I think the most notorious one was John Hinckley Jr., who shot Reagan.)

the legal standard for insanity is so high that few people hit it.
