I'd cross out numbers 1 and 2. Suicide missions and terrorist attacks are usually carefully thought out beforehand. They wouldn't have known much about the flight paths of the target aircraft or how many people were onboard.

Planes lined up for the main runway are spaced at least a mile apart. You can see several at a time at night. That aux runway isn’t used nearly as frequently and accommodates jump planes and private planes. This one qualified for that though I’ve seen plenty land on the main runway, too. They look like small versions of a DC-9. I’ve seen plenty of helicopters taking of from Bolling afb but they tend to hug the ground and stay very low until they’re out of the city. Somebody dropped the ball somewhere between the heli’s altitude and unclear traffic control directions.
