If it was any other human in the world, I'd say it's over the line. Everyone gets old, and incontinence sometimes comes with that.
However, Trump has repeatedly declared himself the most perfect specimen of humanity to ever exist. He has made his own health the topic of discussion. He also constantly makes fun of physical attributes of others, so in this case, turnabout is entirely fair.
It's like when Sarah Palin started talking about how awesome her own rack was. Nobody on the Democratic side talked about her looks before then, but once she made it the topic, it was fair game to say she's not that hot.
The commercial does explain it, why everyone around Trump looks like they're trying not to retch, why Melania doesn't want to be near him. It's that pungent Trump odor, the diaper mixed with overpowering body odor and extreme halitosis. You know Trump is the kind of person who thinks that perfect specimens of humanity don't need to shower or brush their teeth. And of those around him, nobody is going to be the first to tell him he reeks. Reference "The Emperor's New Clothes."
