Accidents happen when someone does something they're not supposed to do or fails to do something they are supposed to do.
Everything else is Force Majeure.

Who was responsible for maintaining the overheated bearings?
Who was responsible for training the train personnel?
Who was responsible for maintaining the tracks?

Answer those questions and you'll have your culprit. the same culprit that has people blaming Biden or Trump rather than accepting responsibility.
