A poor search. Definitely not the first time it's happened. Complacency or flat out laziness. Or just bad luck.

Many tragic stories out there of cops being shot dead by the prisoner in the back seat of the car because someone missed a gun while searching the suspect.

The only way to guarantee safety with a prisoner is total absolute control. But....the public often sneers at methods that accomplish this so often times it's a compromise between officer safety and public acceptance.
