I must admit, well observed.

What happens if Congress/Senate/country/whoever has no confidence in the president, can you hold a vote of not confidence, or is it just impeachment, election etc... way forward?

When it comes to Congress, both politicians and the public give them low approval because:

Democrats think Republican politicians in Congress are bad

Republicans think Democrat politicians in Congress are bad
