It was a three hour sustained attack.. The Liberty was monitoring communications.
Prior to the actual attack which took place around 2:00 PM, Israel monitored Liberty for hours:
"At approximately 0600 hours (all times local) on the morning of June 8, 1967 an Israeli maritime reconnaissance aircraft observer reported seeing 'a US Navy cargo type ship,' just outside the coverage of the Israeli coastal radar defense net, bearing the hull markings 'GTR-5'.[11]
"This report, made to Israeli naval HQ, was also forwarded immediately to the Israeli navy intelligence directorate.[12]

The jamming stories are particularly interesting. The claim every Liberty frequency was jammed proves the attack was deliberate and well planned. Considering the state of the art, especially for airborne jammers, this would have been quite a feat. When I spoke to Captain McGonagle the morning of 9 June 1967, he made no mention of jamming. No other fleet units reported it. We now know an EC-121 was in the area and filed no reports of jamming. When ship's company was able to get the transmitter on the right frequency (see the testimony of Chief Wayne Smith, RMC on 14 June 1967) they were able to get out almost immediately. Some jamming.

Had I been MacNamara, and heard that we'd just scrambled a couple of F4's into a 'hot zone' that nothing but nukes on-board, I'd have told the Navy to get those F4s back to the carrier ASAP, as well.

Now, had they been equipped with anti-aircraft and air-to-surface missiles, I would have let them proceed into the 'hot zone'.

It is the fault of the carrier-commander(s) that they had nothing 'up-top' and ready to launch, which was suitable for an air-interdiction or water-interdiction mission, and, further, their fault, as well, that they either (1) wasted time by launching planes that might be recalled or (2) wasted time by not preparing more suitable birds for launch, once the nuke-armed ones had catapulted off the deck.

Irony is that Seaman Larkins operated Mount 51 was awarded for firing on the torpedo boats in violation of Captain's orders and likely cause the deaths of several of the crew.
US Navy could have prevented all the death and damage if they had told Israel they had the US Liberty in those waters when asked.
Israeli planes fired because the USN assured them there were no USS ships in the area.
Every investigation concluded it was an accident, error of identification.
