First the ballots are handled by human hands to go into the machines to be counted.
Overseen depending upon if there are enough volunteers to see such.
In the case of my State, we have mail-in voting so first the ballot signature has to be checked , then the ballot removed from the envelopes. Once the ballot is removed, traceability is lost. One could easily slip more faked ballots into the pile.
At my county auditor's office where ballots are checked for signature, opened and then counted, there is only one small place of a couple of seats to watch what goes on in a large room of about 2500 square feet and about a dozen workers. Not the ideal nor thorough oversight conditions. Limited to one from each of two parties at a time in the counting room.

By law, both major parties must have representation as observers when opening mail-in ballots and when counting ballots and when reviewing ballots and when duplicating ballots.
And no, you cannot just slip in fake ballots. It's known how many people voted. If you were to slip in fake ballots, you would end up with more votes than voters.
