Heart-wrenching images of a severely emaciated Palestinian boy, spread by leading international media outlets as the âface of hunger in Gazaâ and âproofâ of widespread suffering due to the war, do not tell the full story.
An investigation reveals that the boy, Mohammed Zakaria Ayoub al-Matouq, suffers from multiple genetic disorders and pre-existing medical conditions, a fact entirely overlooked by the vast majority of media coverage.
The chilling image of Mohammed, taken on July 21, 2025, in Gaza, went viral and was disseminated by outlets such as The New York Times, NBC News, The Guardian, and the BBC. The photos, captured by the Turkish government news agency Anadolu, showed Mohammedâs spine protruding from his small back as his mother held him in her arms.
The Guardian captioned the image as âfacing life-threatening malnutrition,â while the UKâs Daily Express described it as a âhorrifying photo embodying the âstorm of human sufferingâ gripping Gaza.â The photographer, Ahmed Jihad Ibrahim al-Arini, told the BBC that Mohammed ârepresents the situation in Gaza.â
But the pro-Israel group HonestReporting was the first to identify a troubling gap in the reporting: The boyâs older brother, Jude, appeared in the background of the photo, looking in much better health. This led to further details that changed the entire picture.
In a surprising CNN report, Mohammedâs mother told the outlet that her son suffers from a âmuscle disorderâ requiring special nutrition and physiotherapy. She noted he was âhappyâ and âable to sit uprightâ when receiving these treatments. Furthermore, pro-Israel journalist David Collier cited a May 2025 medical report from Gaza stating that Mohammed suffers from âcerebral palsy, hypoxemia, and was born with a severe genetic disorder.â

The post, which garnered significant attention, claims the childâs image was used in a âmodern blood libelâ to smear Israel, despite his medical evacuation to Italy on June 12, 2025, coordinated through Israelâs Coordination of Government Activities in the Territories (COGAT) unit.
Osama al-Raqab, whose condition worsened during Gazaâs humanitarian crisis, was evacuated by Israel with his mother and brother via Ramon Airport for treatment in Italy, as confirmed by multiple sources, including the MFA and COGATâs English X account.
Reports from May 2025, including by the Associated Press and CBC News, detailed his severe malnutrition and lack of access to meat, fish, and enzyme tablets critical for managing his cystic fibrosis, a genetic disorder affecting digestion and breathing. These reports, predating his evacuation, highlighted Gazaâs blockade and war-related shortages, with his mother, Mona al-Raqab, noting his weight had dropped to 20 pounds.
