Canada has single payer and people with money still go to a private doctor and parallel system. It would be dumb to try to force these people into the single payer system because a) they wouldn't go, and b) by going private the single payer system has less people to cover.

Today many, if not most people don't have health insurance, they have catastrophic care pretending to be insurance. Anytime you have a deductible so high that only something major means you will get covered... you don't have health insurance.
