In July 2019, the IAEA confirmed Iran had breached both the 300kg stockpile limit and the 3.67% enrichment limit. Then in January 2021, the agency confirmed that Iran began enriching to 20%, and to 60% in April 2021.
So those headline numbers: 4.5%, 20%, 60% were all caught while inspectors still had meaningful access.
In February 2021, Iran halted the more intrusive verification measures required by the JCPOA, such as implementation of the Additional Protocol, daily access to Natanz and Fordow, and continuous surveillance of certain sites.
This is when the picture got murky. The IAEA reported that it "lost continuity of knowledge in relation to the production and current inventory of centrifuges, rotors and bellows, heavy water and uranium ore concentrate, which it will not be possible to restore."
