What specific changes do people think would happen if Trump moved the ideological needle of the supreme court to the right?
I'd expect that if he gets a second term, maybe even in this term, he will have another seat to fill and likely a left leaning one. I'd also expect that like with Scalia and others there will be a vocal argument to nominate someone of similar makeup, but i don't see trump doing that. It would be a center right nominee.
Since conservatives are not inherently constitutionalists based on judicial activism like creating premium rights for incorporations, I'd expect there will be significant changes further empowering concentrated wealth and resources in the private sector over the individual and the different levels of government. I'd also expect a rollback of some civil rights progress, possibly an end to affirmative action. There are many other possibilities in a 6-3 scotus.
