Afterthought. It seems that the discussion assumes that health insurance must exist. I need to ask why that is?
If you were starting a completely new health care system from scratch and you had never seen any health care system anywhere else, would you set up a system involving health insurance companies involved in the system and driven to create profit for shareholder wealth? Or would you look for another model, such as a single buyer which negotiates with providers? There may be other non-insurance models of course.
