What we need is single payer to combat the mafia that health insurance and providers have become.

The state should fund the entire thing.

States can't afford to be the only one providing those services, as it is larger than their entire budget. See: NY and Cal.

By law the states can't operate at a loss like the federal government does.

The program will likely start as an operational loss by the federal government until the costs stabilize, and then we'll see improvement.

Would've kept growing with the current law, but probably more than if we were to implement this proposal. Obamacare does nothing to control insurance costs or the national debt... Just the opposite actually. This new proposal will slowly reduce costs if people are opting into it and hospitals are receiving more funds instead of just 70% of their charges paid by Medicaid and Medicare.

This does not address the earlier point, but I agree with you statements on a stand alone basis.

We can't sustain 9,000 per person per year on average of medical costs.

I don't see why the mandates would further exist if the government says you're free to opt-in or opt-out of the plans stated in the law. I think the Democrats could use that a strong point to gain Republican support, along with not having an employer requirement.

Why is your employer not subsidized?

Because they don't like to provide for their employees. I'm lucky to even make $14/hour through them.

Could put it in the legislation as an amendment.

Not experienced with insurance negotiations with medical facilities, but if the hospitals provided an expense report to Congress, I'm sure the government would know how much money is needed to fund these facilities without them breaking the bank. The expense report would obviously have to be audited by the FDA and Congress.

One would have to look at how much is costs to maintain the equipment, how frequently the equipment needs to be replaced, and how much payroll, utilities, and the hospital cafeteria costs. In fact they can run all of these reports at every major healthcare facility, and then take the average of them and set that as the "hard standard" of what will be paid out... So if some hospitals want more, that sucks for them.

I'd also like to see an amendment that allows people to purchase certain drugs overseas where it'll bring a bit more competition into the equation. For example, I should be able to get 40mg Neximum from Canada if it's not reasonably priced here in the states. 20mg is sold over the counter at a good price, but 40mg is pretty expensive to the point where the insurance I used to have (pretty good insurance) didn't even cover it.

I read the article. I was actually making a point about your specious conception of opting in. Unless you can opt out of being taxed for Medicaid, you're all in, regardless of whether you choose apply for benefits or not.
