They found it because she consistently used her name to donate and ActBlue found it quite easily... She said she didn't know she over paid to one and was more that willing to redesignate when she was informed of the error.

O'Donnell used variations of her own name and apparently her own credit card, not exactly how one acts when trying to commit a crime.

She fixed it. It appears according to the law you have 60 days to correct any over payment to a candidate. Looks like she did just that.

I don't think when committing fraud by using other people's identities to donate that you have the opportunity to fix your over the limit donations because at that point you ....duh.... already fraudulently used someone else's identity.
