How did they know a shooting was going to happen though? What if he was just allowed to carry?

When at church, always assume the person with a gun out is planning to shoot somebody. Do not wait for the sound of gunfire, either get down and evade or attack, depending on situation and ability.

Same thing. Don't pull a gun in church unless going to shoot and prepared to get shot. If you see it out, it's time for immediate action of gaining cover, egress or launching attack, if there is any margin. HINT: Don't stop and pray about it or you might not be around to give thanks later.
