But even in that scenario it ultimately boils down to human error at some level.
I suppose but IIRC this pipe/tube was solid all the way through the area where the crack was so there was no way they could have found it without completely disassembling the whole line which was ultimately how they found it. That was the last possible remediation measure they could try as the inspectors would not clear them for production again until they knew for certain the cause of the contamination. Food processors don't have small machines. It took them weeks. Most of that machinery is driven by high pressure air so I guess the pressure caused the crack.

Where she works now is almost completely automated from silo to the end warehouse. They have little robots that crawl around inside the machines looking for pressure leaks or other issues. She said each of the lines is about as long as a football field. They have to fly in a team to fix any issues with that set up. All the daily workers do is program/monitor the machines, lubricate certain parts, and have meetings about scheduling meetings. Her job is to go to certain stations and load data from them onto an ipad that uploads them to the master production software--basically as a back up to the sensors.
