Trump is doing his part by slashing spending in his budget request particular on social programs because most people don't realize that congress is the one that really establishes the budget, not the president. Republicans in congress are going to have to massage spending on social programs and healthcare legislation so it appears to meet the desires of the far right yet, will have a minimal impact on their states. That's going to be tough because the programs effect all states.

Not sure what you mean by national health care systems. Only about 40 of 200 countries in the world have established any kind of healthcare system. Most of the nations on the planet are too poor and too disorganized to provide any kind of mass medical care.
