Are there any examples from history where these were a success ?

That depends upon how you define success. Theocracies, along other Totalitarian-Authoritarian regimes (Monarchies, Socialists, Communists, Fascists, Dictators) are regimes of Control. They are all faith-based systems used to keep populations under the thumbs of the elite. They are successful until the population rebels. Then it all goes to hell.
Its very depressing. I would quantify success as people living together in harmony with freedom of expression,belief and association. That sort of sounds like a big ask.

That depends upon what you mean by "in harmony".

Freedom includes the freedom to annoy and offend. I've often noted that Tolerance is not Acceptance. We are free to ignore, shun and avoid those with whom we do not wish to associate (consistent with the freedom to be left alone).

So, if in harmony means we just leave one another alone to pursue one's own path, I agree. If it means "I'd like to buy the world a coke" holding hands nonsense, then no.

Define "success".

And I mean that seriously. What standards of measurement are we looking at? Longevity, stability, economic prosperity, what?

By and large, as a Christian and as an American, it is my general opinion that religion and government are focused on very different purposes and goals, and it is probably best that they stick to their own spheres of concern on an official level. But I also recognize that my cultural background probably biases me in this question.

Well, yeah, since by YOUR standard, virtually NO nation in history has been a success.
